Turning Point and Living Well have been commissioned by Westminster, Hammersmith & Fulham and the Royal Borough of Kensington and Chelsea to provide a community-based Health Trainer Service that is free to residents across the three boroughs.
Turning Point and Living Well’s model for the Service is determined and simple: we want to identify those people most in need of guidance and help them achieve their health goals and access appropriate local services.
We want to build partnerships with local organisations, services and communities to provide our clients with personalised and holistic support that will put them in control of their lives and help address health inequalities throughout the three boroughs.
The service is made up of people who have been specifically trained to provide NHS Health Checks and one-to-one Health Trainer support to help empower local people to make positive, informed choices that improve their overall health and wellbeing.
